  • Health information, medical history, and NDIS plan details
  • Medicare number, NDIS number, and other relevant identifiers
  • Financial information for billing purposes

We collect personal information directly from clients, their representatives, or authorised healthcare providers. In some cases, we may collect information from third parties with consent or as required by law

2. Use and Disclosure of Personal Information

We use personal information for the purpose it was collected, including:

  • Providing healthcare services and support under the NDIS
  • Billing, payment processing, and managing accounts
  • Communicating with clients and their representatives
  • Facilitating referrals to other healthcare providers or NDIS services

We may disclose personal information to:

  • Healthcare professionals involved in the client's care
  • Government agencies, insurers, or authorized third parties as required by law
  • NDIS service providers for coordination of support services

We do not disclose personal information for marketing purposes without consent.

3. Data Security

We take reasonable steps to protect personal information from unauthorized access, misuse, loss, or disclosure. This includes:

  • Implementing physical, technical, and administrative security measures
  • Restricting access to personal information to authorised personnel only
  • Regularly reviewing and updating our security practices

Clients have the right to access and request correction of their personal information held by us. Requests for access or correction should be made in writing to our Privacy Officer. We will respond to such requests within a reasonable timeframe and may require verification of identity.

5. Retention and Destruction

We retain personal information for as long as necessary to fulfill the purposes for which it was collected or as required by law. When personal information is no longer needed, we will securely destroy or de-identify it to prevent unauthorised access.

6. Overseas Disclosure

We do not disclose personal information to overseas recipients unless necessary for providing healthcare services, and appropriate safeguards are in place to protect the information.

If you have concerns about our handling of your personal information or believe we have breached the APPs, please contact our Privacy Officer. We take privacy complaints seriously and will investigate and respond to them promptly.
